The first twenty minutes
The bread and the apple both start breaking down into glucose almost as soon as they hit your gut, and that glucose begins slipping into the bloodstream within minutes. The pancreas is watching for exactly this and, in someone without diabetes, starts releasing insulin the moment it senses the rise, often before the blood sugar has climbed very far at all. That early, quick response is one of the things a healthy pancreas does that an injected dose can never quite match, because the injection is always a step behind the food rather than reacting to it in real time.
Where all that sugar actually goes
Once insulin is in circulation, it starts directing traffic. In your muscles it opens a doorway called GLUT-4 and lets glucose in to be stashed away as a fuel reserve for later, a run, a flight of stairs, whatever comes next. In fat tissue it encourages storing the surplus and stops fat from being broken down at the same time. The liver plays by slightly different rules, since it has its own doorway that lets glucose in with or without insulin around, but insulin still tells it what to do with that sugar once it arrives, bank it, turn it to fat, or stop sending sugar back out into the blood. That last instruction matters enormously. Without enough working insulin, the liver keeps releasing sugar into the blood even when there is already too much there, which is a big part of why fasting sugar creeps up early in diabetes.
The feeling of being full is not an accident
Somewhere around the same time, your gut is releasing its own signals, hormones called GLP-1 and PYY, that slow your stomach down and tell your brain you have had enough. The hunger hormone that was nudging you toward lunch in the first place quietens right down. By roughly the two-hour mark in someone without diabetes, blood sugar has drifted back down to where it started, insulin has eased off in step with it, and the whole system settles. In type 1, this is where injected insulin has to do a harder job than the body ever asked of it naturally, because a dose given before the meal has to already be working at the right strength by the time the food's sugar actually lands, and getting that timing right is most of what carbohydrate counting and insulin dosing are quietly trying to solve.
